CDC Ends Universal Hepatitis B Birth Dose: What the New 2025 Vaccine
By ClickUSA News Staff | December 6, 2025 | Health & Wellness
In a historic and highly controversial move, the Centers for Disease Control and Prevention (CDC) Advisory Committee on Immunization Practices (ACIP) has voted to eliminate the universal recommendation for the Hepatitis B (Hep B) vaccine at birth — ending a policy that has been in place for over 30 years.
The decision, passed 8-3 during a tense ACIP meeting on December 5, 2025, restricts the Hep B birth dose to only newborns whose mothers are confirmed positive for the virus or whose status is unknown. For the vast majority of American newborns born to Hep B-negative mothers, the first dose will now be delayed, with vaccination decisions left to “shared clinical decision-making” between parents and doctors.
This marks the most significant rollback of a routine childhood vaccine in U.S. history and comes directly under the influence of Health and Human Services Secretary Robert F. Kennedy Jr.
The New CDC Hepatitis B Vaccine Schedule at a Glance
| Group | Old Recommendation (1991–2025) | New Recommendation (Effective Pending Final Approval) |
|---|---|---|
| Newborns (within 24 hours) | Universal birth dose for ALL infants | Only if mother is Hep B positive or status unknown |
| Low-risk newborns (Hep B-negative mother) | Birth dose required | Birth dose optional; delay to 2 months recommended |
| Infants 1–18 months | 3-dose series starting at birth | 3-dose series starting no earlier than 2 months |
| Unvaccinated children/teens | Catch-up vaccination recommended | Unchanged |
| Adults 19–59 | Universal recommendation | Unchanged |
Why Was the Hep B Vaccine Given at Birth?
Since 1991, every newborn in the United States has received the Hep B vaccine within the first 24 hours of life — regardless of risk. The CDC called it a critical “safety net” because:
- Up to 90% of infected infants develop lifelong chronic Hepatitis B
- Chronic infection dramatically raises the risk of liver cancer and cirrhosis
- The birth dose protects against rare hospital errors, undiagnosed maternal infection, and future household exposure
- The U.S. reduced new infant infections from ~20,000 per year to under 20
The vaccine (Recombivax HB or Engerix-B) has one of the longest and strongest safety records of any immunization.
RFK Jr.’s Direct Role in the Change
As HHS Secretary, Robert F. Kennedy Jr. dismissed the entire previous 17-member ACIP panel earlier in 2025 and replaced it with appointees widely viewed as vaccine-skeptical, including figures known for questioning established vaccine science.
Critics say the new panel’s vote reflects ideology over evidence. During the December meeting, members raised concerns about aluminum in vaccines, potential links to allergies, and “immune overload” in newborns — claims repeatedly debunked by decades of global research.
Experts Warn: “We Will See Hepatitis B Come Back”
Public health leaders reacted with alarm:
- American Academy of Pediatrics: Strongly opposes the change and continues to recommend universal birth dose
- Infectious disease specialists predict a rise in preventable chronic infections
- Some hospitals have already announced they will maintain universal vaccination regardless of the new guidance
The vote still requires final approval from acting CDC Director Dr. Jim O’Neill or Secretary Kennedy himself, expected before the end of 2025.
What Parents Need to Know Right Now
- If your baby is born to a Hep B-positive or unknown-status mother → Birth dose remains mandatory and urgent
- If the mother tests negative → You and your pediatrician will now decide whether to vaccinate at birth or wait
- Insurance coverage and state school requirements have not yet changed, but battles are expected in 2026
This decision is the first domino in what many fear will be a broader overhaul of the childhood vaccine schedule under the current administration.
